Case Manager

General Job Description:
Still Waters Professional Counseling is seeking a Case Manager to work with children, adolescents, and families in Columbia, Burke, McDuffie, and Richmond Counties.  Travel is reimbursed.  Case manager duties include assessing, planning, implementing, monitoring and evaluating actions required to meet the individual’s health and human services needed. Case Management experience, a plus.  This is a full-time position.

Job Responsibilities:

  • Coordinate and provide care that is safe, timely, effective, efficient, equitable, and individual-centered.
  • Handle case assignments, review case progress and determine case closure.
  • Work on skill building with individuals.
  • Help individuals achieve wellness and autonomy.
  • Facilitate multiple care aspects (case coordination, information sharing, etc).
  • Help individuals make informed decisions by acting as their advocate regarding their clinical status and treatment options.
  • Develop effective working relations and cooperate with medical team throughout the entire case management process.
  • Coordinate arrangements with social services, health and governmental agencies.
  • Take the extra mile and interact with individuals to keep track of their progress and to ensure satisfaction.
  • Record case information, complete accurately all necessary forms.
  • Promote quality interventions and outcomes.
  • Assess and address motivational and psycho-social issues.
  • Adhere to professional standards as outlined by protocols, rules and regulations.
  • Will maintain 25 Billable hours per week (full-time equivalent).
  • Other duties as assigned.

Requirements:

  • Proven working experience in case management in mental health care manager, nursing, or a related job
  • Excellent knowledge of case management principles

Medical Assistant

General Job Description:
To perform this job successfully, an individual must be conscientious, courteous and have excellent customer service, organizational, and computer skills. 

Must work in a busy front desk environment, have attention to detail, be adaptable and energetic, doing various tasks pertaining to the processessing of individuals into behavior health service in an electronic health record system. 
Must also be willing to take on other tasks as needed to assist.

Perform administrative and certain clinical duties under the direction of a physician and Still Waters Administrative staff. Administrative duties may include scheduling appointments, maintaining medical records. Clinical duties may include taking and recording vital signs and medical histories, preparing clients for examination and administering medications as directed by physician.

Job Responsibilities:

  • Schedule client appointments for Psychiatrist.
  • Follow-Up calls and confirm appointments
  • Fill-In last minute appointments as needed
  • Manage Psychiatrist schedules, altering as needed
  • Prepare and monitor timesheet or sign-in sheet for Psychiatrist.
  • Prepare treatment rooms for client sessions.
  • Greet and log in clients arriving at office or clinic with assistance from other front desk personnel
  • Show clients to session rooms and prepare them for the physician.
  • Interview and record clients’ medical history, vital statistics (such as: vital signs, weight and height), or information such as test results in medical records.
  • Assist in explaining treatment procedures, medications, diets, or physicians’ instructions to clients.
  • Input Pharmacy Information in Electronic Health Records
  • Handle follow-up problems with Medications
  • Do prior authorizations for medications
  • Perform general office duties, such as answering telephones, or completing insurance forms.
  • Contact medical facilities or departments to schedule clients for tests or admission.
  • Other duties as needed.
